What do I need to submit to request Continuity of Care (CoC)?
Before submitting the required documentation for Continuity of Care (CoC) review, double-check you meet the initial qualification criteria. If you're unsure, you can always give Member Care a call at 855-282-0822.
Here's a breakdown of what you might need to submit when requesting CoC:
- Appointment details: Date of your upcoming (treatment visit) related to your current active course of treatment. Remember, it must be within 30 days of your Sidecar Health coverage effective date.
- Recent medical history: Copies of your doctor's notes (who is recommending this treatment) from your last 3 visits.
- Good faith estimate (GFE): A detailed breakdown of the expected cost for your upcoming service, including any discounts and separate charges for the doctor, facility, lab fees, etc. Your provider will know this as a “good faith estimate”.
- Service details: Where your appointment is scheduled (hospital, clinic, etc.) and the names of the doctor and other providers involved.
If your care is ongoing, related to maternity care or mental health care, you’ll also need to include the following:
- Ongoing care: If you've received similar care before, include a statement (Explanation of Benefits/ EOB) from your previous insurance along with the cost estimate for your upcoming service.
- Maternity care: For pregnancy, provide your estimated due date, delivery method (vaginal, C-section, etc.), how far along you were when you started your Sidecar Health coverage (weeks/months), and your chosen delivery location and OBGYN's information.
- Mental health care: Documents showing you've seen a licensed therapist or counselor at least once a month for the past 6 months, or a statement (EOB) from your previous insurance for similar services with your current provider's name (CoC won't apply if you want to see a different therapist).
Important tips:
- Make sure all documents are complete and unedited.
- Avoid submitting letters from your doctor's office requesting CoC on your behalf.
