Using your Sidecar Health plan at Knox Community Hospital (for Ohio Eastern Star Home employees)

Office visits, labs, and imaging are covered at cost. Your deductible may apply, but you’ll pay nothing if it’s been met or doesn’t apply to the care you’re receiving. See the next section for more details on how to access office visits, labs, and imaging.

Steps to getting (covered) office visits, labs & imaging services at Knox Community Hospital:

1. Get care
2. Pay with your Sidecar Health VISA benefit card. They may bill you later, which is okay.
3. Call KHC for your itemized invoice [phone] or [phone].
4. Upload your itemized invoice and "Knox Community Hospital PLA letter" (distributed by your HR team) to your member portal expense.
   • Important note: Including your PLA letter ensures no out-of-pocket costs beyond your deductible (expires 12/31/2024). Contact your HR department or Sidecar Health Member Care if you didn’t receive the letter.  

Steps to getting (covered) procedures, surgeries, etc. at Knox Community Hospital:

1. Call Member Care at [phone] with care details and cost estimates from KCH (if you have them).
2. We'll estimate coverage and out-of-pocket costs. If the estimated cost is higher than your coverage, we'll search for nearby providers with lower rates for the same service.
3. We’ll follow up in ~5 business days which will help you make informed decisions about your care.
4. Schedule care, get care, and pay per standard Sidecar Health process.
